Enhanced detection of cortical lesions in multiple sclerosis using magnetization transfer and double inversion recovery

چکیده
Purpose: Multiple sclerosis (MS) has long been considered a pure white matter (WM) disease. In recent years, greater attention is paid to cortical pathology in MS. However, detection of cortical lesions is complicated by low contrast on MRI between lesions and surrounding tissue, the relatively low image resolution, and the confounding signal from WM and cerebrospinal fluid (CSF). Double inversion recovery (DIR) simultaneously suppressed the signal from CSF and WM, greatly enhancing the detection of cortical lesions. However, the number of lesions detected by DIR is much lower than detected on histology. We propose to use magnetization transfer (MT) in combination with DIR (denoted MT-DIR) for improved detection of cortical lesions in MS.

3 Tesla and 7 Tesla MRI of multiple sclerosis cortical lesions.
Cortical lesions are prevalent in multiple sclerosis but are poorly detected using MRI. The double inversion recovery (DIR) sequence is increasingly used to explore the clinical relevance of cortical demyelination. Here we evaluate the agreement between imaging sequences at 3 Tesla (T) and 7T for the presence and appearance of individual multiple sclerosis cortical lesions. Eleven patients with...

Introduction: Evidence from pathological studies continues to underscore the significant involvement of cortical grey matter (GM) in multiple sclerosis (MS) [Stadelmann, C., et al., Curr Opin Neurol, 2008. 21(3):229-34]. Although some of this cortical pathology is now detectable as a result of advances in sequence development (e.g., double inversion recovery), contiguous areas of subpial demyel...

The relevance of cortical grey matter pathology in multiple sclerosis has become increasingly recognised over the past decade. Unfortunately, a large part of cortical lesions remain undetected on MRI using standard field strength. In vivo studies have shown improved detection by using higher magnetic field strengths up to 7 T. So far, a systematic histopathological verification of ultrahigh-fie...

However, detection of these lesions with conventional MR imaging remains a challeng...

BACKGROUND In current clinical practice, T2-weighted magnetic resonance imaging (MRI) is commonly applied to quantify the accumulated multiple sclerosis (MS)lesion load, whereas T1-weighted sequences are used to differentiate edema, blood-brain barrier breakdown by contrast enhancement, and irreversible brain tissue damage(commonly called “black holes” owing to the loss of signal intensity in T...
